A 53-year-old female; chief complaint blurry vision; radial keratectomy (RK) 30 years ago
EXAM FINDINGS
Entering distance vision: OD 20/25, OS 20/25 at distance; best-corrected distance vision: OD -0.25-0.75x120 20/25+1, OS plano -1.25x 070 20/25+1, +2.00 add 20/20 OU; stromal scars from RK
APPROACH
Anterior segment OCT with epithelial thickness mapping (ETM)
DIAGNOSIS
Normal ETM is between 50 um to 59 um. Patient had several areas of increased ETM, explaining complaints of reduced vision.
MANAGEMENT
Re-normalizing the ocular surface with a scleral lens that has appropriate central clearance, limbal clearance and landing zone characteristics
