PATIENT

A 67-year-old Black male; chief complaint: not seeing well through his left eye, with a black spot that comes and goes; ocular fatigue

EXAM FINDINGS

Entering distance VA: OD 20/20 +0.75-1.25X115; OS 20/20-1 +0.25-0.75X071; stable macular drusen OU with SN; large dark area

APPROACH

Examination of the peripheral retina with the aid of UWFL photo and referral to retinal specialist

DIAGNOSIS

Retinal detachment (RD) superior nasal OS with suspicion for melanoma

MANAGEMENT

Referral to retinal specialist to rule out melanoma vs. RD; diagnosis: macula on RD with retinal tear at 10:30 (seen next day; surgery three days later)

PATIENT CARE TIP

Prompt referral to specialist to facilitate surgery
